ADULT WAIVER & PARTICIPANT RELEASE FORMS MUST BE Brought with you the day of the event June 24, 2017.


The Midwest AMP Fest Event Admission is $30 per person. Includes event programming, dinner and snacks, T-Shirt and goodie bag.

Registrations made after June 4 will include a meal, but may not include a shirt and all giveaway items due to ordering time.

Full payment for all reserved spots is due at the time of registration. We accept credit card payment only . Checks/cash will not be accepted. No monies will be accepted the day of the event.

The Midwest AMP Fest Event will only accept group registrations . No Individual registrations will be accepted.

Youth must be teens who have just graduated 6th grade, are currently in junior high or high school, or graduated high school in 2017. Youth may not register themselves and all youth must be accompanied by adult chaperones. Chaperones must be at least 21 and have completed the training their home diocese requires to work with youth.

Chaperone/Youth Ratio: Each group must have 2 chaperones per 1 - 10 youth. A group may be as small as 2 adults and 1 teen. If a group is larger than 2 chaperones and 10 teens then they must have 1 to 2 chaperones per 10 youth after the first 10. Groups may not exceed 2 adults per 10 youth registered.
